There are three main sheets. PDF versions are attached to this page below.

  1. Part 1: A page that needs copying to the line leader of the subject to complete on the subject, a couple of peers and someone the subject directly manages.
  2. Part 2: A page for the leader (the subject) to complete about themselves
  3. All pages should be returned to the person doing the evaluation (often the line leader of the subject)
  4. All information returned should be treated in confidence to encourage frankness and honesty.
  5. For completeness a feedback form is provided to plan growth points
